With Pharmacies Shutting Down, Maryland Residents Drive Longer Distances for Medications

The closure of pharmacies, such as the one in Hancock, Maryland, poses significant challenges for patients, especially in areas where access to medication is already limited. With over 525,000 residents in Maryland living in pharmacy shortage areas, many face long travels—often exceeding 10 miles—to find alternative pharmacies. This situation is exacerbated when patients encounter issues like insurance acceptance and establishing care at a new location.

Pharmacist Ken Reed highlighted that the reason behind his closure was unsustainable reimbursement rates from pharmacy benefit managers (PBMs). PBMs, which play a crucial role in determining how much pharmacies are compensated for prescriptions, have been criticized for shortchanging pharmacies, making it financially unviable for many independent ones to remain open.

While Reed and others advocate for legislation to guarantee minimum reimbursement rates that cover both drug costs and dispensing fees, there are concerns that this could potentially lead to higher costs for patients. Lobbyists for PBMs argue that increased reimbursements would have to be offset by higher premiums or out-of-pocket costs for consumers.

However, the experience in West Virginia, where similar reforms were enacted, suggests that it’s possible to raise reimbursements without negatively impacting insurance premiums. In fact, a recent review showed that such changes did not lead to an increase in costs for consumers.

Despite previous attempts to address reimbursement issues in Maryland, proposed bills have struggled to advance, leaving many pharmacies vulnerable. As the healthcare landscape evolves with alternative options like mail-order prescriptions, stakeholders emphasize the need for diverse pharmacy access, ensuring that patients have crucial local options rather than being reliant solely on mail services.

The complexities of pharmacy reimbursements, patient access, and the role of PBMs create a challenging environment that needs ongoing attention to ensure patients receive the medications they need without significant barriers.
